Authority to Construct/Permit to Operate (AC/PO) Application Form
AC/PO Application Cover Form (this form is REQUIRED for all applications)

The Application for Authority to Construct - Permit to Operate cover form was updated on 9/17/2010. Please make sure to use the new form. Application submittals using the old form will be returned.

The application cannot be processed until the filing fee has been received.
Effective July 1, 2009:  The application filing fee has increased to $121.00.
The rate for evaluation/inspection fees has increased to $74.00/hour.

Before submitting the AC/PO application, please check to see if the proposed process has an associated supplemental form. If it does, please fill out the associated supplemental form and attach it to the application.
